Intro
Signature Aftercare supports young people aged 16–18 who are preparing to leave local authority care and transition into independent living.
Our vision is to become a trusted provider of supported and independent living services, known for delivering consistent outcomes, maintaining high standards, and supporting young people to achieve lasting independence.
We work closely with local authorities, placement teams, and referral partners to ensure that each placement is appropriate, well-supported, and aligned with the young person’s pathway plan.
In the UK, young people leaving care are entitled to structured support, accommodation, and guidance to help them move safely into adulthood, including housing, education, and life planning support.
Primary Group We Support
Young People Leaving Care (16–18)
We primarily support young people who:
- Are currently in care or transitioning from care
- Have been placed under local authority responsibility
- Are preparing for independent living
These young people often require:
- Stability
- Consistent support
- Guidance in developing life skills
Our service aligns with the wider purpose of leaving care services, which are designed to support young people in improving life outcomes and successfully transitioning into adulthood

Transition Stage Individuals
Our service is particularly suited for young people who are:
- Approaching the age of independence (17–18)
- Preparing to move into their own accommodation
- Requiring a gradual transition rather than immediate independence
Without structured transition support, many young people leaving care struggle with housing stability and independence. Supported environments help them learn essential skills such as budgeting, cooking, and managing responsibilities before moving on.

What we do not position as
To maintain clarity and safety, our service is not designed for:
- Direct public access placements
- Self-referrals without professional involvement
- Individuals outside of supported accommodation eligibility
All placements are managed through appropriate referral and assessment processes.
