Making a Referral
We work closely with local authorities and professional partners to provide a clear, structured, and responsive referral process.
Intro
Signature Aftercare accepts referrals from local authorities, placement teams, and professional partners for young people requiring supported accommodation.
We recognise that every referral represents a responsibility. For this reason, our process is designed to be clear, structured, and aligned with professional standards, ensuring that each placement is appropriate, safe, and outcome-focused.
All placements are made in collaboration with referring professionals and are subject to assessment and suitability.
Who can make a Referral
We accept referrals from:
- Local Authorities
- Placement Coordination Teams
- Social Workers
- Residential Care Providers
- Other Care Sector Professionals
Referrals must be made by authorised professionals to ensure that each placement is appropriate and aligned with the young person’s needs.
Making a Referral
Step 1: Initial Referral
All placements are made in collaboration with referring professionals and are subject to assessment and suitability.
- Background and current placement
- Risk assessments
- Support needs
- Any existing care or pathway plans
In supported accommodation services, referrals typically include detailed information to ensure a full understanding of the young person’s needs before placement decisions are made.
Step 2: Review & Assessment
Once a referral is received, it is carefully reviewed. This includes:
- Assessment of suitability
- Consideration of risk factors
- Review of support needs
- Evaluation of placement type
Local authorities are required to ensure that placements meet the individual needs of each young person and provide a safe and appropriate environment.
Step 3: Matching Process
If the referral progresses, a matching process is undertaken to ensure that:
- The environment is suitable
- The placement aligns with the young person’s pathway plan
- Support levels are appropriate
Matching is a critical part of the placement process and ensures safety for both the individual and others within the accommodation.
Step 4: Placement Agreement
Once suitability is confirmed:
- Placement terms are agreed
- Funding is confirmed by the local authority
- Move-in arrangements are scheduled
In many cases, placement decisions are made through structured processes involving local authority teams and professional review panels.
Step 5: Transition & Move-In
The young person is supported through a structured transition into the service. This includes:
- Introduction to the environment
- Initial engagement
- Support planning
Our focus is to ensure a stable and positive transition from the outset.
